I died twice in a game, but it never showed up in my death count in the list. Just noticed it the second time.
and here is a screen shot where the show is circled with red circle From the second game, and yes the first game also encountered this bug.
 
Not even sure how it can be replicated and/or what causes it. Hopefully this helps.

If you don't know how internet speed works then here is a small lesson: In order to make your ping go higher you just have to use internet for a lot more things other than just playing a game. For Example: Downloading/Torrenting, Watching videos on multiple devices or having a lot more devices connected to the same network and all those devices actively using the internet. 
How hard is it for you to grasp that concept?On a side note, I'm pretty sure there are more than 10 ways of making your ping go higher.
That can only be fixed by adding a ping limit and once that limit is crossed the player should be kicked out of the server. This has been suggested multiple times by the community. It would be even better if Server Creators are given the option to specify a custom ping limit but if they fail to do so the game itself use a ...    less
